Sport Physicals

To participate in any school-sponsored athletic program, all student-athletes in Sedalia School District 200 must have a current physical examination on file before attending practices, tryouts, or games. This policy ensures the health and safety of all participants and is in accordance with Missouri State High School Activities Association (MSHSAA) guidelines.

Physical Requirements
    •    A completed MSHSAA Pre-Participation Physical Evaluation Form must be submitted prior to participation.
    •    Physicals are valid for two years from the date of the exam unless otherwise noted by a physician.
    •    Annual completion of the MSHSAA Medical History Form is also required.

PHYSICAL EXAMINATION FORM:  These are good for two years from the date of the last physical examination (as long as so marked by physician).  You can find this form under Quick Links.   

 PRE-PARTICIPATION DOCUMENTATION:  Every year students must complete the MSHSAA Pre-Participation Documentation form.  This page is three pages in length and  located under Quick Links.   This form is due once a year and is good from July 1 to June 30 of each year.  Please indicate if you have insurance.  The school does purchase a plan to cover athletes during athletic/activities events. However, it does only provide basic coverage.
